It does not take much to sign your name. Yet, in the little time it does take, you could save lives. That’s right … just by signing your name to the Anatomical Gift Statement on the back of your Wisconsin driver’s license. A donor card will also work, if you don’t have a license. And when you sign it, be sure to tell your family.

Three easy steps to become an organ and tissue donor:

Step 1: Indicate your request on your driver’s license or donor card:
  • Check the box for “any needed organs and parts.”
  • Or write what specific organs and tissues you would like to donate.
Step 2: Sign your name. If your driver’s license is laminated prior to your filling out the anatomical request portion, just write over the lamination. Place an orange “donor sticker” on your license, too.
Step 3: Most importantly, inform your family of your donation decision. Share your life. Share your decision.
